Location and Parking

The clinic is located in the vibrant Sydney Road precinct and is 7 km north of the Melbourne CBD. It is on the right approximately 30 metres after Moreland Road on the corner of Sydney Road and Cozens Street, when heading towards the city on Sydney Road.

Parking is available for patients and staff in the side streets surrounding the clinic as well as in the railway reserve at the end of Cozens St. The clinic is also easily accessible by Public Transport via the Sydney Road (No 19) and Moreland Road (No 8) Trams and by Train (Moreland Train Station on the Upfield Line).

After Hours Care

We provide 24-hour care for our patients. If you have an urgent medical problem when the clinic is closed, please ring the clinic number on 9923 7888 and you will be provided with the contact details for the accredited medical deputising service used by the clinic – National Home Doctor Service (NHDS). Alternatively you can call NHDS directly on 13 SICK (137425).

If you have an emergency you should dial 000 and request the Ambulance Service.

 

New Patients

Please call the clinic on 9923 7888 for further information regarding registering as a new patient of the clinic. 

The clinic welcomes new patients. However, we ask that you arrive 15 minutes early for your appointment to complete the required paperwork and bring your medicare card with you. You can also choose to complete the paperwork at home by printing out and completing the following form before attending the clinic.

Management of Personal Health Information

Summit Medical Group collects information about its patients to provide on-going health services of a high quality. Your medical record is a confidential document. It is our firm policy to maintain privacy and security of personal health information at all times, and to ensure that this information is available only to authorised members of our staff. Communication Policy

If you feel you need to contact our GPs by phone, please call the clinic. A detailed message will be taken by the reception staff and forwarded to your doctor, who will return your call at their earliest convenience or when they are next consulting at the practice. Our practice does not charge a fee for telephone communication. Patients are unable to email the GPs in our practice directly. However, they are able to contact the administrative office via email to forward correspondence to the GPs where required. This email address is provided to patients on request by practice staff. Every effort will be made to respond to emails within 48 business hours of receipt. Please note that it is practice policy that queries of a medical nature will not be dealt with via email. Kindly make an appointment with your doctor to discuss these. In addition, appointments cannot be made or changed via email.
